TEACHERS:
Complete this form only if your school is holding a science fair before March 3, 2007 and you wish to submit the winning entries to the state fair. In this case individual students should not send preliminary abstracts. They must, however, submit final abstracts on the proper form by the March 3 deadline (Rule 8).

Personal Data: Data collected on Preliminary Application Forms are considered public. Information may be supplied to sponsoring organizations, participating schools, and the press.
